The driver of a Tesla Model 3 that killed a 76-year-old woman in Texas had manually overridden the car's Full Self-Driving system by pressing the accelerator to 100%, the National Transportation Safety Board said on July 15.
Michael Butler was driving on a residential street in Katy, Texas, with a 30 mph speed limit when the vehicle struck a home at more than 70 mph on June 19, killing Martha Avila. The NTSB's preliminary report found that Butler had engaged Tesla's driver-assistance feature but then overruled it moments before the crash.
Butler has been charged with manslaughter in connection with Avila's death.
