Brazilian aviation investigators have concluded their inquiry into the August 2024 Voepass airliner crash, finding that ice accumulation, a faulty de-icing system and crew missteps were central to the disaster that killed all 62 people aboard near Vinhedo, São Paulo.
The final report, released on July 23 by the Cenipa accident investigation centre, identified 19 contributing factors to the crash of the ATR 72-500. Investigators cited ignored cockpit alerts and an organisational culture of "normalisation of deviations" at Voepass. The aircraft had ten items with known faults before takeoff, according to the report.
Brazil's civil aviation authority, Anac, suspended Voepass operations in March 2025 and revoked the airline's operating certificate in June 2025.
